General Power of Attorney

A General Power of Attorney (GPoA) allows someone you authorize in advance to make business, financial and legal decisions for you. Without this document, if you are mentally incapacitated, or otherwise unable to make your own decisions, you may be unable to access your bank accounts, pay important bills, or take necessary steps to for your own support and maintenance.
Healthcare Power of Attorney

A Healthcare Power of Attorney (HPoA) allows someone you authorize in advance to make healthcare decisions for you in the event you are unable to do so (e.g. you are unconscious). Without this document, it may be unclear what your wishes are, who should make these critical decisions, and family members can end up in arguments if they disagree on what’s best.
Living Will

A living will expresses your desires with regard to end-of-life decisions. Do you want to be put on life-support, or wish to avoid dialysis or blood transfusions? These are difficult decisions for your family during a highly stressful time, so having a clear understanding of what you want is invaluable. Living wills are often used in conjunction with a HPoA.
HIPAA Authorization

The Health Insurance Portability and Accountability Act (HIPAA) is a federal law designed to protect your private medical information (including mental health). Most medical providers will not disclose your medical info to anyone else without this authorization. For example, a doctor won’t discuss medical condition of a child over 18 with the parent.
